1. Send us your photos using our convenient form below. Take extra care to ensure the pictures are as clear as possible.
2. Main Street’s dedicated treatment coordinators will retrieve the photos and share them with our Orthodontist. Then, we will prepare an estimate*, including treatment options, insurance coverage, length of treatment etc. and will contact you back.
3. Upon discussion of your estimate*, you can schedule an appointment with office.

* Estimates are not final until you meet with a Main Street Orthodontist, review medical records and additional records and create final treatment plan.

How to take your virtual smile consultation photos:

• Take 5 close-up shots (use guide below).
• You can use a tongue depressor or a spoon  to retract cheeks
• Bite NATURALLY.
• Save and upload photos along with patient information and insurance details into the secure form provided.
